Medical director supervision

Sculptra and HA filler are both done under the supervision of a medical director. I'm the licensed RN doing the injection, and I follow physician-directed protocols. My medical director has 23 years of practice and about 12 years in aesthetics. You can look up my license on the North Carolina Board of Nursing site.

Some people use both: HA filler for one defined area and Sculptra for the overall picture. I'll tell you whether that makes sense for you or whether one is enough.

The trade-off that matters most

The FDA notes that fillers not made of materials such as hyaluronic acid can be difficult or impossible to remove. Put plainly, HA filler can be dissolved, and Sculptra can't. If the idea of something you can't undo makes you uneasy, start with HA.

Cost

Pain and downtime

Pressure and quick pinches. Some fillers contain lidocaine, a numbing medicine. Plan on some swelling and maybe a bruise for a few days; most people carry on with their day.

How long it lasts

The FDA says PLLA effects generally show up gradually over several weeks and may last up to 2 years. For HA, the FDA says it depends on the material and the area.

Your first visit

Bring the question as it is: "which one?" I'll look at your face from the front and side, at rest and moving, and walk you through the table above with your face in mind.

Is it safe?

The FDA's page on dermal fillers lays out the risks. Hold off if you're under 22, since the FDA approves dermal fillers for adults 22 and older, or if you're pregnant or breastfeeding, because the FDA says filler safety there is unknown. If the skin where we'd treat is inflamed or infected, including acne, a rash, or hives, we reschedule. Tell me about a bleeding disorder, severe allergies or past anaphylaxis, a lidocaine allergy, or a tendency to form keloids or raised scars.

The FDA lists bruising, redness, swelling, pain, tenderness, and itching as common dermal filler side effects and says many clear up within days to weeks. The most serious risk is filler getting into a blood vessel. The FDA calls that chance low, but it can cause tissue damage, vision problems including blindness, or stroke. Get care right away for unusual pain, any change in your vision, or skin near the injection site that turns white, gray, or blue.
